First Appointment

When you arrive at our office the first time, its our pleasure to make you and your family feel right at home. Our staff will have several questions for you so that we can do a thorough evaluation of your children's health. We utilize Phreesia Pads to electronically register your family and enter all info into our Amazing Charts Electronic Medical Records System. To assist with a smooth registration process please bring the following with you:

• Parent's valid driver's license /military ID or some valid state issued I.D.
  

• Children's shot records /copies of medical records from other physicians offices/ hospitals ( if you don't have these we can request them for you with your written authorization).
  

• Valid insurance card
  

• Write down any questions or concerns you may have so that you don't forget to bring them up with the practitioner.
  

It's very important for us to have all this information as soon as possible. This ensures that we will be able to provide the very best service possible to you and your children.
